Hemisection

Tooth 46 – mesial root with apical inflammation

A hemisection is a surgical procedure usually performed at the lower molars for the conservation of a root. The roots are separated with a mill. One root is then removed. Due to the opening of the pulpal cavity during root separation, the remaining root must be provided with a canal filling and possibly supplied with a pin-stump structure. The remaining root serves as cantilever bridge for further prosthetic treatment.

Further indications for a hemisection are:

  • Massive subgingival root caries
  • Extensive parodontitis profunda at one root only
  • Tooth fractures, in which one root is still worthy of conservation
